Maniac's Heart is a Beyond mode Emblem.
Strategy
All stages have 
Serrated mod, it is recommended that you have 
Thought Bubble in your heirloom slot, and obtain it immediately in your inventory. This will prevent the Sawblades from staying on the screen.
Drop Condition
When you kill Strangers in the Basement with 
Maniac's Mask, there is a chance you will obtain Maniac's Heart (was about 1/10 for me).
When you get inside basement, it is recommended that you kill every monsters, then use either 
Loudhailer or 
Handbell combined with 
Event Horizon + 
Astral Spike to spawn them. Then, equip Maniac's Mask in order to kill them in the basement. Otherwise, you will only end up rescuing them.
Killing strangers with any other method while not wearing Maniac's Mask does not work.
The mask has the "kill" tag, so if you use the 
Bookworm Mastery and get the 
Book of Mastery(the legendary book) and 
Book of Combat plus get some candy going, you should be able to use the Bookworm ultimate to get this. I'd say it's comfortably more than 5% odds. I recommend keeping the last bit of your inventory full with candy/bones/etc so you don't run out of inventory space early.
Notes
The Basement spawns at the 2nd half of the quest during the Halloween event, making this a limited time access Emblem.
